The Details:

• All visits must be used within one year from the date of purchase.

• The plan may only be extended in the event of an “acute care” injury. At that time you will be placed on the regular fee schedule until the time that your injury becomes maintenance again.Your plan will be extended for the same amount of time that you were injured.

• The Wellness Plan cannot be returned, exchanged, or used by more than one person. No exceptions.

• The Wellness Plan cannot be used in conjunction with any discounts or offers.

• The Wellness Plan can be used every 14 days for maintenance adjustments.
